Things to know about CBT
Before you start
Start your CBT with someone new gently as you can always work up to rougher stuff.
Make sure the person on the receiving end can signal when it's getting too much.
During CBT play

Tied up genitals should be checked now and again. If numb or cold, circulation is being cut off and it's time to untie.
When circulation's cut off, eventually the person won't feel pain. Prolonged loss of circulation can cause damage that can't be reversed. Ideally ties should be loosened every half hour and never left on for long periods or over night.
Genitals tied up for too long can swell above and beyond a normal erection. If untied quickly any swelling should go down.
EMT (Emergency Medical Technician) or bandage scissors are useful in case you need to untie someone's cock or balls quickly: they won't cut the genitals as you cut him free. These are available online from medical suppliers.
When playing with cock rings, metal ones have one major drawback: they can't be taken off an erect dick until it goes soft again. It's possible to put one on a soft cock that, once erect, turns out to be too big but the ring can't be removed. Adjustable cock rings are better and leather ones can be cut off.
Best avoided
When either man is under the influence of drink or drugs, CBT is a bad idea. These deaden pain, and pain is a useful indicator of when to stop before damage is done.
It's a bad idea to tie someone's cock or balls to something else (a wall, post or other part of their body). If they respond to strong stimulation by a sudden movement they'll yank hard on their genitals, risking injury.
When medical help's needed
Hard blows to an erect cock can actually break it (a 'crack' can be heard). This injury is serious and without urgent medical help the harm can be permanent, leading to loss of function. An ice pack (such as a bag of frozen peas wrapped in a tea towel) pressed on the area will help stop bleeding and reduce swelling while you get to a doctor.
Haematoma is when blood vessels in the cock break and a pool of blood forms under the skin, causing a hard, bulging blood clot. If this is large or keeps growing get medical help quick.
Hit hard, balls can rupture. Their lining splits, spilling the contents out into the ball sack. This causes extreme pain, and internal bleeding causes massive swelling of the ball sack, turning it black and blue. Get to a hospital at once or else the ball could be lost forever.
The same symptoms of pain and swelling happen with testicular torsion, which is when the chords that the balls hang down into the ball sack from become twisted. Again, get to a hospital quick.
